What is Form 8822?

Form 8822, or officially Change of Address, is an Internal Revenue Service (IRS) form that U.S. taxpayers use to notify the federal tax agency of their change of address.

Since the IRS sends correspondence, documents, and even checks, to taxpayers by mail, after moving to a new residence, it is the responsibility of taxpayers to notify the IRS to avoid miscommunication and any problems caused by not receiving important documents.

The federal tax agency uses the address listed in the most up-to-date tax return of a taxpayer when sending mail; thus, it is important for taxpayers to update the address they have on file when they move using Form 8822.

How to Fill Out Form 8822?

Form 8822 is a single-page that requires personal information used to change the home mailing address of a taxpayer. You may write “NA” if a question is not applicable unless otherwise directed.

Part I

Items 1 and 2
Mark all the checkboxes that your change of address affects. Mark Item 1 if it affects Individual income tax returns, including Forms 1040, 1040A, 1040EZ, 1040NR, and so on. Mark Item 2 if it affects gift, estate, or generation-skipping transfer tax returns, including Forms 706, 709, and so on.

For Forms 706 and 709-A, provide the name of the decedent and social security number.

Item 3a
Provide your full legal name.

Item 3b
Provide your social security number.

Item 4a
Provide the full legal name of your spouse.

Item 4b
Provide the social security number of your spouse.

Item 5a
Provide your prior names. Use this item if you changed your name due to legal reasons, including marriage or divorce.

Item 5b
Provide the prior names of your spouse. Use this item if your spouse changed his or her name due to legal reasons, including marriage or divorce.

Item 6a
Provide your old address. If a foreign address, provide the name, province or country, and postal code of the foreign country.

Item 6b

Item 7
Provide your new address. If a foreign address, provide the name, province or country, and postal code of the foreign country.

Part II

In the space provided, you may provide your telephone number.

The “Sign Here” section requires your signature. Also, provide the date when you signed the form.

If a joint return, your spouse should also sign the form and provide the date of signing.

If a representative, executor, or administrator filed the form, he or she should also sign and provide his title and the date of signing.
