Before entering the establishment, individuals need to fill out a Contact Tracing form to know if they carry the disease or not. Through this, establishments can keep a record of who visited the premise and can conduct possible actions in case of COVID-19 detection.

What is a close contact?

A close contact refers to a person who may have acquired the virus through:

1. Face-to-face contact with an infected person within 1 meter for more than 15 minutes;
2. Direct physical contact with a confirmed or probably infected person;
3. Direct care for a patient with a confirmed or probable case without proper safety precautionary measures; or
4. Situations that generate higher risks of virus transmission.

What are the affecting cases of close contact?

1. Confirmed COVID-19 case. It refers to any individuals who tested positive for COVID-19 through a certified laboratory confirmation.
2. Probable COVID-19 case. The Probable COVID-19 cases fall under three conditions. It refers to any individuals whose testing is inclusive, those who tested positive for COVID-19 in an unauthorized testing center, and those who died without confirmatory testing.
3. Suspect COVID-19 case. Any individual who has the following conditions fall under the Suspect COVID-19 case.

a. The Severe Acute Respiratory Infections (SARI) case has no other etiology that elucidates the clinical presentation.

b. An Influenza-like Illness (ILI) case has no other etiology that elucidates the clinical presentation and has a travel history to an area with reported COVID-19 transmission. Also, an individual has close contact with a confirmed or probable case.

c. Individuals with morbidity, assessed as having a high-risk pregnancy, aged 60 years and above, or a health worker showing respiratory signs fall under suspect COVID-19 cases.

What is the purpose of a Contact Tracing form?

A Contact Tracing form helps track down people who are at risk of being exposed to the virus. This form includes demographic and geographic profiles, possible health conditions, and other questions regarding COVID-19.

While Contact Tracing aims to reduce the spread of infection and stop transmission from occurring, a Contact Tracing form supplements the goal of a successful contact tracing.

A Contact Tracing form helps notify close contacts of the possibility of infection and extends preventive care. Also, it helps health forces apprehend the epidemiology of a disease in a specific area.

Using a Contact Tracing form helps track down areas where possible transmission may occur. It helps private or public establishments to keep a record of individuals who visited the premises. Hence, they can alert individuals who might be at risk if they track down a COVID-19 case.
