DA Form 67-10-2 is a Field Grade Plate Officer Evaluation Report (OER) used by the U.S. Army. It's essentially a performance review form for commissioned officers in the ranks of Major (O-4) and Lieutenant Colonel (O-5), as well as Warrant Officers in the grades of Chief Warrant Officer 3 (CW3) to Chief Warrant Officer 5 (CW5).
