What is Form DA 2028?

Form DA 2028, Recommended Changes to Publications and Blank Forms, is a document used by the US Army to request a change in publications and sponsored forms by the Army Publishing Directorate (APD). The proponent of this form is the Office of the Administrative Assistant to the Secretary of the Army.

Using the Army DA Form 2028 falls under AR 25-30, Army Publishing Program and anything that is released following AR 25-30 can be changed using the DA 2028 Form including AR 25-30.

Army publications, otherwise known as DA publications, are documents published under two different agencies or commands that are authenticated by the Department of the Army. These publications will remain in effect indefinitely unless it is superseded, canceled, or rescinded by the US Secretary of the Army. Digital copies of these publications are kept on the official website of the APD, for anyone who wants to use or reference these publications.

It should be kept in mind that Form DA 2028 requires the proponent that published the publication you are requesting to change. Generally, publications will have that information on the form ready for you to refer to.

Aside from these publications, the DA 2028 Form can also be used to request changes for Repair Parts and Special Tool Lists as well as leave remarks on how to improve publications.

Submitting the DA Form 2028 is different depending on what you are filling out the DA 2028 Form for. If you are requesting a change on a publication, submit the form to the Publication Control Officer or Forms Control Officer of the proponent organization. The email addresses for the PCO and the FMO of proponent organizations can be found on the APD website.

If you are leaving comments and suggestions to help improve certain publications, then refer to the publication itself and look for the paragraph or clause on suggestions. These clauses often have information on how they want to be contacted, and how they want the DA 2028 Form sent.

Input the name of the organization that is the proponent of the publication. Then, input the complete mailing address of that organization, including their ZIP Code. You can identify the proponent of the publication when you look up the information about the publication on the official APD website. From there, you will be able to research the address of the proponent organization.

FROM:

Input your complete name and address. If you are a soldier, identify the location you are in and for what purpose you are there.

Part I - All Publications

Publication or Form Number, Change Number

Input how the publication is referred to on the official APD website.

Publication or Form Date

Input when the publication was written and made public. This information is found on the information page of the form on the official APD website.

Title

Input the official name of the publication or form. Do not abbreviate anything. This information is found on the information page of the form on the official APD website.

Comments

You can have multiple comments about the publication you are filling out form DA 2028 for. However, you must include all that is applicable from the following:

• Comment Number

Each comment must be distinguishable from one another. Each specific change must be labeled in a different comment.

• Work Package Number or Data Module Code

If the publication has a work package number or data module code (more commonly known as a QR code) make sure to add it when commenting about the publication.

• Page number

Indicate the page number where the content you want to comment on is found.

• Paragraph number

Indicate the paragraph number within the page the content you are commenting on is found.

• Line number

Indicate the line in the paragraph on the page you want to comment on.

• Figure Number

If you are commenting on a figure displayed in the publication, indicate the figure number.

• Table number of the table you want to comment on

If you are commenting on a table displayed in the publication, indicate the table number.

• Recommended change

This is your actual comment. What needs to be changed in the portion of the publication you have listed?

• Reason for changing

Justify your proposed change in a concise yet substantial set of sentences.

If you do not want anything to change in the publication, and instead you want to suggest things that can improve feature iterations of the publication, fill out Part III instead of Part I.

Part II - Repair Parts and Special Tool Lists

Publication or Form Number, Change Number

Input the number that is assigned to the list.

Publication or Form Date

Input when the list was written and published.

Title

Input the official name of the list.

Comments

You can have multiple comments about the list. However, you must include all that is applicable from the following:

• Comment Number

Each comment must be distinguishable from one another. Each specific change must be labeled in a different comment.

• Work Package Number or Data Module Code
