The ITD 3239 form, otherwise known as the Idaho Change of Address Form, is a form submitted to and issued by the Idaho Transportation Department. This form is usually filled out by those seeking to change both the address written on their driver’s license and vehicle registration as required by the Idaho Code.
If you are seeking to renew your driver’s license and vehicle registration address, then you are eligible to fill out the ITD 3239 form. Note that if you are looking to purchase a new driver’s license, identification card, or registration, you can visit your county’s local Department of Motor Vehicles (DMV) or equivalent.
To fill out this form, you will need to input all information that is required from you in this form.
Specifically, you will be needing your full, legal name, your former name if you have legally changed your name beforehand, your Idaho driver’s license number or identification card number, your daytime phone number, and your date of birth. You will also be asked to provide the license plate number of up to four (4) vehicles currently in your possession.
You will also be needing your current physical address, otherwise known as your current residential address, the city in which it is located, and the zip code. You will also need to indicate your mailing address in the event that it is different from your physical address. For this, you will need the city, state, and zip code of your mailing address. You will also be asked to verify this information by inputting the date of signing as well as affixing your signature in the designated space.
Additionally, if there are any additional residents in your household who also request a change of address, then you may input all of their information in the designated space. You will be needing his or her full legal name, former name if he or she has previously changed their name beforehand, Idaho driver’s license number, daytime phone number, date of birth, license plate number for up to four (4) vehicles currently in his or her possession, the date of signing and whether he or she wants to make a change to his or her current physical address, mailing address if different from the current residential address, or both. Finally, his or her signature must be placed within the designated space. The single ITD 3239 form can account for up to two (2) additional residents to a change of address.

Before attempting to fill out the ITD 3239 form, please ensure that all information you are about to provide and input into the form is completely accurate and truthful. This is a legal document used by the state of Idaho and thus should be treated with the utmost care and attention.
This section will ask mainly for the personal information required to process this form.
At the top left of this section of the form, you are asked to input your full legal name with no specified format. You will also be asked to write down your former name if you have previously undergone the process of legally changing your name.
The line below the previous one will ask you to indicate either your Idaho driver’s license number or your identification card number. You will also be asked for your daytime phone number otherwise known as the phone number that can reach you if called between the hours of 9:00 AM to 5:00 PM. Finally, for this line, you will be asked to write down your full date of birth in no specified format.
The final line of this section asks you to indicate your license plate number of up to four (4) vehicles currently in your possession. If you have multiple vehicles, you are not asked to list them down in any particular order.
This section asks for your updated residential and mailing addresses as well as additional information regarding these.
The first line of this section asks you to indicate your current physical address, otherwise known as the address in which you currently reside. You will also be asked to indicate in which city in the state of Idaho you live in as well as the corresponding zip code.
If you currently have a mailing address that is separate from your current physical address, you will be asked to indicate this in the following line. For your mailing address, you will be asked to indicate in which city and state that the mailing address is located. Indicate also the corresponding zip code. To finalize all information in this section, input the date of signing, once again with no specific format, as well as your signature in the indicated space.
As previously stated, you are allowed to change the address of up to two (2) other individuals who are requesting the same address change shown above.
For each resident, he or she must indicate his or her full legal name, followed by his or her former name if they had previously undergone the process of legal name change.
In the following line, he or she must write down his or her Idaho driver’s license number or identification card number. Following this, he or she must write down his or her daytime phone number, or the number used to contact them between the hours of 9:00 AM and 5:00 PM. Finally, for this line, the additional resident is asked to write down his or her date of birth in no particular format.
He or she must also list down the license plate numbers of up to four (4) cars currently registered under his or her name. He or she must also indicate whether they want to make changes to the physical address, mailing address if separate from their current residential address, or both. To finalize everything, the date of signing must be indicated as well as the signature of the additional resident.
Once everything is finished, congratulations, you have successfully completed the ITD 3239 form.
