Date

Enter the date that this form is being sent to the tenant's previous landlord.

Tenant’s Name

Enter the name of the tenant in question.

Requestor

Enter the name of the requestor.

Rental Unit Location

Enter the address details of the rental unit.

Fax Number and Email

Enter the fax number and email that the requestor uses to receive completed Rental Verification forms.

Move-in and Move-out Date

Enter the date that the Tenant moved in and the date that they moved out.

Monthly Rent and Inclusions

Enter the amount in rental payments made every month, and any inclusions to the Rent.

Rent Payment - On Time

Mark the box that indicates how often the tenant paid their rent on time. Check only one of the following:

• Always
• Sometimes
• Never

If you check “sometimes”, enter the number of times the payment was late, and the longest period of time (in days) that rent payment was late.

Occupants and Rules

Enter the number of occupants in the property, and indicate if they followed the rules or not.

Checks - NSF

Indicate if any checks were returned as NSF. If you check “yes”, enter the number of NSF checks returned.

Security Deposit

Mark the relevant box to indicate if the security deposit was returned. If you check “no”, explain why in the space provided.

Complaints and Property Condition

Indicate if there were any complaints made about the tenant(s) by other tenants, and if the property was kept in good condition. If you check “yes”, explain why in the space provided.

Reason/s for Vacating

Enter the reason or reasons for why the tenant vacated this property, then mark the relevant box to indicate if prior notice was given or not.

Relation to Tenant

Mark the relevant box to indicate if you or anyone in your company (if applicable) is related to the tenant.

Rent Again

Mark the relevant box to indicate if you would ever allow the tenant to rent your property again. Then explain why or why not in the space provided.

Person Verifying
