What is MA License Application Form?

The State of Massachusetts Driver's License Application form is a document used to apply for a Learner's Permit, Driver's License, or Massachusetts ID Card.

If you already have a driver's license, you can renew it up to 1 year before its expiration day. The State of Massachusetts does not send out renewal notices and you have to keep track of the expiration date of your license. If your license has been expired for more than 2 years, you will take the learner's permit exam and road test.

How to fill out the MA License Application Form?

1. Service Type

1. Type

Select the type of ID you are applying for(Real ID or Stand ID).

2. Document to Issue

Select the document to be issued(Learner’s Permit, Driver’s License, or Massachusetts ID card).

3. Class of Learner’s Permit/License

If you operate any combination of vehicles with a gross combination weight rating (GCWR) of 26,001 or more pounds, provided the GVWR of the vehicle(s) being towed is more than 10,000 pounds, select "A".

If you operate any single vehicle with a GVWR of 26,001 or more pounds or any such vehicle towing a vehicle, not above 10,000 pounds GVWR, select "B".

If you operate any single vehicle or combination of vehicles that do not meet the definition of Class A or Class B, but is designated to transport 16 or more passengers including the driver, select "C".

If you operate any single vehicle or combination of vehicles that are typical passenger vehicles such as cars, SUVs, or family vans, select "D".

If you operate a motorcycle, select "M".

4. Service Type

Select the type of service you need (New, Renewal, Replacement, Out-of-State Conversion, Reinstatement, CDL downgrade, or Change of information). If you will change information, select from the listed choices (Name, Address, DOB, Gender, Height, or Eye Color).
