Form MV-44, also known as Application for Permit, Driver License or Non-Driver ID Card?, is issued by the New York State Department of Motor Vehicles (DMV). It is used to apply for or renew a learner's permit, driver's license, or non-driver ID in New York.
Type of Application
Check the appropriate box on whether you are applying for a driver license, permit, or ID card.
Purpose of Application
Check the appropriate box regarding the purpose of your application: “New,” “Renew,” “Update Info,” “Change Type,” “Replacement,” “Conditional,” “Restricted,” “Transfer to New York”
If you have ever had a New York driver license, learner permit, or non-driver ID card, check “Yes” and provide its ID number. Otherwise, check “No.”
Provide your full legal name.
Existing Driver License
If you have ever had a driver license that is valid or that expired within the last two years, issued by another US State, the District of Columbia, or a Canadian Province, check “Yes” and provide the place where it was issued, its expiration date, the type of license, and its out-of-state license ID number. Otherwise, check “No.”
Date of Birth
Provide your date of birth.
Check “Male” if you’re male. Check “Female” if you’re female.
Provide your height in feet and inches.
Provide your eye color.
Provide your telephone number and include its area code.
If your name has changed, check “yes” and provide your former name as it appears on your current license or non-driver ID card.
If applicable, provide the change in your new driver license or ID card as well as the reason for the change.
Social Security Number (SSN)
If you have been issued an SSN, provide your SSN. Otherwise, check the box below.
Provide the address where you receive your mail. Include the street number, street name, apartment number, city or town, state, zip code, and country.
If your residential address is different than your mailing address, provide your residential address. Include the street number, street name, apartment number, city or town, state, zip code, and country.
If you are a veteran and you would like to have “Veteran” printed on the front of your photo document, check the box and provide proof of your honorable discharge from military service.
New York State Organ and Tissue Donation
If your would like to be added to the New York State Donate Life Registry as an organ donor, check “Yes” and sign and date on the space provided. Otherwise, check “Skip This Question.”
If you would like to donate $1 to the Life...Pass It On Trust Fund for organ and tissue donation research and outreach, check the box provided.
Voter Registration Questions
If you are not registered to vote where you live now and would like to apply to register, check “YES” and if you’re not submitting this form to a DMV office, complete the voter registration application section. Otherwise, check “NO.”
Registrations with the US Selective Service System (SSS).
If you wish to not register for the SSS, check “NO.”
1. If your driver license, learner permit, or privilege to drive a motor vehicle been suspended, revoked, or canceled or has your license application been denied in this state or elsewhere, check “Yes.” Otherwise, check “No.” If you answered “Yes” to the preceding question answer the following question:
a. Has your license, permit, or privilege been restored, or has your application been approved? If yes check “Yes” and if no check “No.”
2. If you have received treatment, currently receive treatment, or take any medication for any condition that causes unconsciousness or unawareness, check “Yes.” Otherwise, check “No.”
3. If you need a hearing aid and/or a full view mirror to drive a motor vehicle, check “Yes.” Otherwise, check “No.”
4. If you have lost the use of a leg, arm, hand, or eye, check “Yes.” Otherwise, check “No.”
a. If you need to renew your driver license, checked “Yes” on the previous question, and the injury occurred since your last driver license, check “Yes.” Otherwise, check “No.”
b. If you checked “No” to the previous question and your condition has gotten worse since your last driver license, check “Yes.” Otherwise, check “No.”
Check the appropriate box regarding whether the form is for a junior license or a non-driver ID card.
If you are applying for either, provide the signature and relationship to the applicant of a parent or guardian as well as the date of signing the space provided.
Teen Electronic Event Notification Service (TEENS)
If you would like to enroll in the TEENS program, provide the ID number on a New Tork State driver license, permit, or non-driver ID card of the consenting parent or guardian that was provided above.
Commercial Driver License Applicants Only
1. If in the past 10 years a driver license was issued to you from another state in the US or the District of Columbia, check “Yes” and provide the name of each state. Otherwise, check “No.”
2. Check the appropriate box you wish to certify to DMV the commercial motor vehicle that you operate or expect to operate:
a. Non-excepted Interstate (NI)
b. Non-excepted Intrastate (NA)
c. Excepted Interstate (EI)
d. Excepted Intrastate (EA)
