What is the Florida Duplicate Title Application Form?

Form HSMV 82101, Application for Duplicate or Lose in Transit/Reassignment for a Motor Vehicle, Mobile Home, or Vessel Title Certificate, or also referred to as the Florida Duplicate Title Application Form, is used by Florida residents to apply for a duplicate certificate of title for their motor vehicles, mobile homes, or vessels.

Florida residents may request a duplicate title in case their original titles were lost, stolen, or damaged. If their original certificate of title happens to be damaged, applicants must submit the original copy along with the Florida Duplicate Title Application Form.

How to fill out the Florida Duplicate Title Application Form?

Applicants can download and print a PDF copy of the Florida Duplicate Title Application Form from the Florida Department of Highway Safety and Motor Vehicles (DHSMV) website that they can manually complete. They can also fill out the Florida Duplicate Title Application Form electronically on PDFRun.

To fill out the Florida Duplicate Title Application Form, you must provide the following information:

• Vehicle/vessel lost in transit - There will be no fee required if your motor vehicle application has been made within 180 days from your last title issuance date and has been lost in mailing.

Mark the “owner” box to verify that you are the one applying for a duplicate title for your motor vehicle or vessel.

Lienholder Date of Lien

Enter the date when the lienholder has issued the loan to you.

Motor Vehicle Mobile Home Number, Recreational Vehicle Dealer Number, or Auction License Number

Enter the Motor Vehicle Mobile Home Number, Recreational Vehicle Dealer Number, or Auction License Number of your motor vehicle.

The dealer or auction license number does not apply to vessels.

Lienholder or Dealer or Auction Name

Enter the full legal name of your lienholder, dealer, or auction.

License Plate or Vessel Registration Number

Section 5. Lienholder Information

If no lien, print “None”

Enter the word “none” if your motor vehicle, mobile home, or vessel is not attached to a lien.

Mark the appropriate box which corresponds to the type of information you will be entering on this form about your lienholder. You may select:

• Federal Employer Identification Number (FEIN) - Enter the federal employer identification number (FEIN) of your lienholder.

• Driver’s License Number, Sex, and Date of Birth - Enter the driver’s license number (DLN), sex, and date of birth of your lienholder.

• Department of Motor Vehicles (DMV) Account Number - Enter the Department of Motor Vehicles (DMV) account number of your lienholder.

Date of Lien

Enter the date when the lien was issued by the lienholder.

Enter the full legal name of your lienholder.

Lienholder Authorization

Mark the box and countersign if your lienholder has authorized the Department to send the duplicate title to you; otherwise, leave it blank. If you did not mark the box, the duplicate title will be mailed to your lienholder.

Odometer Reading Condition

Mark the appropriate box which corresponds to the condition of the odometer reading. You may select:

• Is in excess of its mechanical limits
• Is not the actual mileage

Mark the box if you certify that the motor vehicle, mobile home, or vessel described on this form will not be operated on the streets and highways or waterways of this state and no Florida license plate has been transferred to or purchased for this motor vehicle.
