What is the Nevada Driver’s License Application form?

To drive legally in the State of Nevada, a resident must have a valid driver’s license.

Form DMV 002, Application for Driving Privileges or ID Card, or the Nevada Driver’s License Application form is a legal, state-specific form Nevada residents use to apply for a driver’s license. Aside from its use as an application form, it is also used to apply for a renewal or replacement of Nevada licenses and permits.

If you are not a resident of the State of Nevada, you cannot use the Nevada Driver’s License Application form.

Other Information

In this section of the form, you will be asked to provide a series of answers. Denote your answer by marking the appropriate box and writing the required information when asked of you.

Specifically, you will be asked if:

  • You want to update your address for voter registration purposes
  • You moved to a different country
  • You have a United States Armed Forces honorable discharge and would like to have this indicated on your license
  • You have ever served on active duty in the Armed Forces and separated from such service under conditions other than dishonorable
    • If you select yes, you are authorizing the DMV to send your personal information to the Department of Veterans Services to provide benefits information to you
  • You have ever been assigned to duty for a minimum of 6 continuous years in the National Guard or a reserve component of the Armed Forces and have separated from such service under conditions other than dishonorable
  • You have ever served the commissioned corps of the United States Public Health Service or the Commissioned Corps of the National Oceanic and Atmospheric Administration of the United States in the capacity of a commissioned officer while on active duty in defense of the United States and separated from such services in conditions other than dishonorable
  • You would like to register for selected service
  • You would like to be an organ donor
  • You have ever had a license or identification card under a different name
    • If you answer “Yes”, then you will have to provide the name which you previously went by.
  • You have ever had a license or identification card in another state
    • If you answer “Yes”, then you will have to provide the state where you got the license or card.
  • You will also be asked if the card is in your possession
    • If yes, you will have to provide the license number, class type, and expiration date
  • You have ever had your license revoked, suspended, canceled, or denied
    • If yes, write down the state, date, and reason
  • You have any disability, illness, missing extremity, or take any medication that could affect your driving ability
    • If yes, you will have to provide an explanation.

Affidavit

In this section of the form, you will be asked to verify all information previously placed.

If you are a minor, you must obtain your parent’s written consent. They have to write their name in the appropriate space.

If you would like to be an organ donor, your parents will also have to be informed of that.

You will also have to note that you have not operated a vehicle since the last date of operating a vehicle.

To certify all the information in this form up to this point, you will have to place your signature, your parent’s signature if you are under the age of 18, the date of signature, and the DL/DAC/ID number.

Voter Registration

You can also use this form to update your voter registration information.

You first need to clarify whether you are a US citizen and if you will be 18 years of age by Election Day.

You will also have to provide your phone number and email address but these are not required for the purpose of completing this form.

You will also have to indicate your party registration, choosing among the following:

  • Democratic
  • Independent American
  • Libertarian
  • Nonpartisan
  • Republican
  • Other
    • You will be asked to provide the appropriate name
