What is the Ohio Vehicle Registration Form?

Form BMV 3774, Applications for Certificate of Title to a Motor Vehicle, or also referred to as the Ohio Vehicle Registration Form, is used by the residents of Ohio to request a certificate of title for their motor vehicles. An applicant may request for an original title, duplicate title, replacement title, memorandum title, or a salvage title.

Lien Holder

Enter the full legal name of the motor vehicle’s lienholder. If your motor vehicle has no lien, enter “none.” if the motor vehicle has more than one lien, attach statements of all of the motor vehicle’s liens along with this Ohio Vehicle Registration Form.

Code Number

Enter the code number of the motor vehicle’s lien.

Address

Enter the mailing address of the motor vehicle’s lien holder.

Duplicate Certificate of Title

You are required by the law to state the true selling price of the motor vehicle. If you entered a false statement, you will be violating section 2921 13 of the Ohio Revised Code and will be punished by being imprisoned for 6 months. Furthermore, you may also be fined up to $1000.00.

Sales or Use Tax
