What is Form 735-6438?

If you moved to a different address within Oregon, you are mandated to notify the Oregon Department of Motor Vehicles (DMV) about your address change. To do so, you would have to apply for a change of address by filling out the Form 735-6438 or the Oregon Change of Address Notice for DMV Records.

Why do I need to update my address?

Your driver’s license, permit, or identification card counts as a valid identification. A valid ID is considered a primary identification and is required in applying for government benefits and services. It is also useful in conducting business transactions to avoid fraud.

A valid ID serves as proof of your identity. It should primarily show your legal name, address, and date of birth. It could also have any other additional information about you, depending on the type of ID you are applying for. Therefore, it is important that the information on your ID is updated. Having false or non-updated information on your ID may be considered as a crime and may result in you being fined or imprisoned.

When you update your address to the DMV, the DMV will change your address on all their records: (1) your Driver License, Permit, or ID card record; (2) your vehicle registration records*, and; (3) your voter registration record (this is optional).
*Updating the DMV of your address change would update the address of all vehicles registered in your name.

How to fill out Form 735-6438?

Form 735-6438 is two pages long. The first page is where you’ll input your personal information. The second page contains more information that you have to know about changing your address on your DMV records.

Some reminders before filling out the form:

• Read through the form carefully before filling out to avoid mistakes when writing.
• Answer the form in PRINT (all letters must be capitalized).
• Remember to provide ALL the information needed in the form. DMV may not be able to update your address if you left any item in the form unanswered.

Personal Information
The form will ask you to provide the following information:

• Official Driver’s License, Permit, ID, or Customer Number
• Date of birth
• Full legal name
• New Residence Address (Make sure to input your complete address of residence)
• New Mailing Address (Write here your complete mailing address; if you have no new mailing address, simply write your existing mailing address in the space provided)

Voter Information
When you submit this form, you also consent to having your voter registration updated. You may opt to not have your record updated by checking the box provided in the section for “Voter Information”.

When do I have to submit Form 735-6438?
You must submit your Change of Address Notice Form within 30 days of changing your address.

Will I be getting a replacement ID card or an address sticker?
The submission of an accomplished form only entails an update of your records in the DMV. You will not be issued a replacement ID nor an address sticker reflecting your current residence address.

• Starting January 1, 2020, the DMV has stopped issuing address stickers for driver’s license, permit, or IDs cards. Those who wish to have their address reflected in their ID must apply for a replacement of the DL, permit, or ID cards.

How do I apply for a replacement card?
If you would like a replacement card, you can apply online or you can schedule an appointment to apply. The application fee for the replacement cards are as follows:

• License or Permit: $26.00
• ID Card: $39.50

Does changing my address with DMV also update my address with USPS?

• DMV does not update your address with the US Postal Service. Visit the USPS website for more information on how to file a change of address.
• Similarly, changing your mailing address with the USPS does not change your mailing address with the DMV.

If I move out of Oregon, do I still have to notify the Oregon DMV?
If you are from Oregon and you wish to update your information on your DL, Permit, or ID card when you move out of Oregon, you do not have to submit an accomplished Form 735-6438. Kindly visit your state’s DMV office to know about their process for change of address.

What if I’m new to Oregon?
If you have recently moved to Oregon, you would have to transfer your out-of-state driver’s license to the DMV. When applying for a new license, prepare for the following documents:

• Proof of your name
• Proof of legal US presence
• Proof of Oregon address
• Your social security number
• Application fee
