What is the TA Crash Report Form?

The TA Crash Report Form, more commonly known as a Texas blue form or CR-2 form, is a Texas Department of Transportation (TxDOT) form that is completed and signed by the driver of the vehicle involved in a crash. If the driver is unable to complete the report, another person may complete the report on behalf of the driver, with an explanation as to why the driver was unable to complete the form.

The TxDOT collects crash reports for car accidents in Texas involving more than $1000 in damages, or that result in the death or any level of injury to those involved. Crash reports need to be filed with TxDOT within 10 days of the accident.
