What is the Utah Vehicle Registration Form?

• Small motor vehicles; enter the measurements in cubic centimeters and horsepower
• Motor home; enter the length in feet and inches and name the motor vehicle’s class
• Off-highway vehicle; mark the appropriate box which corresponds to the type of off-highway vehicle you have. You may select:
  • All-terrain vehicle (ATV)
  • Motorcycle
• Street-legal all-terrain vehicle (ATV)
• Low-speed vehicle
• Snowmobile
• Manufactured housing
• Heavy truck; enter the registered weight of the heavy truck. Provide your U.S. Department of Transportation Number (USDOT) if your truck weighs 10,001 pounds or more.

Year

Enter the year or model cycle that your motor vehicle is in. This is defined by the 10th digit of its vehicle identification number (VIN).

Enter the make of your motor vehicle.

Model

Enter the model or name of the motor vehicle product.

Farm Use

Enter YES if your motor vehicle is used for farm activities; otherwise, enter NO.

Vehicle Identification Number

Enter the vehicle identification number (VIN) of your motor vehicle.

Enter the number of cylinder engines your motor vehicle has.
