What is the Delaware Change of Address Form?

The Delaware Change of Address Form is issued by the Delaware Department of Motor Vehicles (DMV) and is used by eligible applicants to notify the DMV of any change in the applicant’s address for their Delaware driver's license and their vehicle registration.

You must notify the DMV and provide the street address of your new residence within 30 days of changing your address. You may not enter a post office box or business address as long as the address that you will enter must also be located in Delaware.

Tips:

  • Aside from mailing the form, you may choose to change in person at the Delaware DMV office.
  • You cannot change your address online. Same with the mailing process, your new address will show on your records, but you’ll need to request replacements if you want your new address to show on your driver’s license/ ID card, vehicle registration and certificate of title, and other documents.
  • There are many forms released by the Delaware DMV online so you could handle your DMV-related requests and applications from home.
  • If you need to request a duplicate or make any corrections on your vehicle title such as vehicle weight change, use Form MV 213 (Application for Duplicate, Corrected, Weight Change Title)
  • If you need to apply for a vehicle registration renewal or if you are having your vehicle inspection conducted out-of-state, use Form MV210 (Out of State Vehicle Inspection Report)

  • If you need to change or make corrections on a vehicle title with a lien, you need to use the Form MV35 (Title Application With Lien).
  • If you failed the vehicle emission inspection, you need to take your vehicle to the car repair shop and present the Form MV-705 (Vehicle Emissions Repair Report ) to record any emission-related fixes that have been made.
  • If you were required to take an eye exam to get your driver’s license, submit Form MV-312 (Report of Visual Status by an Optometrist or Ophthalmologist).

  • If you need to change your name, within 30 days after legally changing your name, you must go to an office of the Division of Motor Vehicles to change the name on your driver’s license. Before heading to the DMV office, you must report to the Social Security office and provide any document that verifies your name change. The document must be original because copies are not accepted. If the document is not in English, a certified English translation must also be submitted with the original document.

Wait for 72 hours after updating your information with the Social Security office before reporting to the DMV office. Bring your old driver’s license and all name change documents.

  • This form is for changing address only. If you want to change your gender, you may do so in person by bringing an original certified court order or an amended birth certificate verifying the change.
  • For changing of address, your license will be replaced by the DMV at no cost.
  • If you are registering your vehicle, Delaware law requires certain equipment. The following are required equipment: headlights, taillights, parking lights, license plate lights, stop lights, turn signals, reflectors, foot brake, parking or emergency brake, windshield and windows, windshield wipers, rearview mirrors, muffler, horn, and seat belts.
