What is the Delaware Driving Record Request form?

Form MV-703, Personal Information Release Form, or referred to as Delaware Driving Record Request, is an official document in Delaware used to request the release of an individual’s personal driving or vehicle records from the Delaware Division of Motor Vehicles (DMV).

A Delaware driving record shows a person’s driving history. It includes an individual’s personal information such as:

• Driver’s license number
• Driver’s license status
• Convictions
• Moving Violation points
• Medical or disability information

How to fill out the Delaware Driving Record Request form?

The Delaware Driving Record Request form is a two-page document. Do not leave the required items blank to avoid any problems with your request.

Note: If you’re requesting on behalf of another person, complete either the Release by Consent of Individual section or the Release Authorized by Statute section. Failure to provide either will result in the denial of your request.

Enter the date when you signed this form.

DMV Account Number

Enter your DMV account number.

Company Name

Enter the full legal name of the company.

Address

Enter the complete address of the company including the City, State, and ZIP code.

Name of Requester

Enter the full legal name of the requester or representative, if the requester is not present, in capital letters.

Requester’s Home Address

Enter the complete home address of the requester in capital letters.

Requester’s Driver License Number

Enter the driver’s license number of the requester.

DL State

Enter the state where the driver’s license is registered.

Signature of Requester

Affix the signature of the requester.

The signature of the requester must be notarized if he or she will not appear in person.

INQUIRY ON:

Name

Enter the full legal name of the driver in the following format: Last Name, First Name, and Middle Name.

Date of Birth

Enter the birthdate of the driver.

Delaware License Number

Enter the Delaware license number of the driver.

Complete this section if you’re requesting driving records.

Plate/VIN Number

Enter the plate or Vehicle Identification Number (VIN) number of the driver.

Complete this section if you’re requesting vehicle records.

RELEASE BY CONSENT OF INDIVIDUAL:

Enter the “Inquiry On” name in capital letters.

Enter the full legal name of the authorized person or requester in capital letters.

Signature

Affix the signature of the driver.

RELEASE AUTHORIZED BY STATUTE

Mark the appropriate box or boxes to provide additional information to be released by statute. You may select:

• For use by a government agency, including any court of law, enforcement agency, or any private person or entity acting on behalf of a government, in carrying out its functions (Section 305(b)(1)).
• For use in the normal course of business by a legitimate business or its agents, employees, or contractors, but only:
  • To verify the accuracy of personal information submitted by the individual to the business or its agents, employees, or contractors; and
  • If such information as so submitted is not correct or is no longer correct, to obtain the correct information, but only for the purpose of preventing fraud by pursuing legal remedies against or recovering on a debt or security interest against the individual.
• For use in connection with any civil, criminal, administrative, or arbitration proceeding or pursuant to any court order.
• For use by any insurer or insurance support organization, or its agents, employees, or contractors in connection with claims investigation activities, anti-fraud activities, rating, or underwriting.
• For use in providing notice to the owners or lienholders of towed or impounded vehicles.
• For use by a licensed private investigative agency or licensed security service.
• For use by an employer or insurer to obtain information relating to a holder of a Commercial’s Driver License.

If you choose the first or second option, enter the name of the business, its address, and telephone number.

If you choose the third option, enter the case caption, civil or criminal docket number, and purpose.

If you choose the fourth, fifth, sixth, or seventh option, enter the name of the organization, its address, and telephone number.

Note: Delaware DMV does not release personal information for marketing purposes.

Tips:

If you are a representative of a company or business and have a valid reason to obtain another person’s driving or vehicle records, fill out the entire form and have it notarized. You must also comply with the following:

• You must have a notary witness upon signing this form. Let him or her sign and affix the notary’s seal or stamp.
• Attach a letter drafted and printed on business letterhead containing the: date, name and address of the business, individual’s name and license number of whom the request is for, and a live signature.
• Submit the completed Delaware Driving Record Request form, business letter, and check or money order made payable to DMV for $25 per inquiry via mail.

If you are requesting your own driving or vehicle records, complete the followings sections in the Delaware Driving Record Request form:

• Date
• Name of requester
• Requester’s home address or current mailing address
• Requester’s driver’s license number and state of issuance
• Signature on requester’s signature line
• You must have a notary witness upon signing this form. Let him or her sign and affix the notary’s seal or stamp.
• Complete Inquiry On section
• Specify if you want to obtain a three-year, five-year, or complete driving record and enter it across the top of page one of this form.

After completing the Delaware Driving Record Request form, submit it together with a check or money order made payable to DMV for $25 per inquiry via mail.
