Form DL 1P, otherwise known as the Virginia Driver’s License Replacement form, is issued by Virginia’s Department of Motor Vehicles (DMV). Residents of Virginia use it to apply for a replacement of their driver’s license, learner’s permit, state identification card, hearing impaired ID card, or emancipated minor ID card.

What do I need to fill out the Virginia Driver’s License Replacement form?

To fill out Form DL 1P, you will be needing your application information, which includes your full legal name, birth date, social security number, gender, race, height, weight, eye color, hair color, current residential street address, apartment number, city, state, ZIP code, name of city or county of residence, former name (if previously changed), and your mailing address if different from your current residential address.

Additionally, if you are under the age of 18, then you also need the written consent of your parent/guardian and, if you have been found not innocent of any offense in a Juvenile and Domestic Relations court, then you also need the consent of a judge.

If you wish to put a special indication on your driver’s license, permit, or ID card, such as insulin-dependent diabetic, speech impairment, hearing impairment (license only), intellectual disability, or autism spectrum disorder, you will need to submit a physician’s statement.

You also need to prepare $5 for the service fee for transactions conducted in the Customer Service Center (CSC) if the transaction is eligible to be performed by internet or mail, unless the renewal is conducted with another transaction that must be completed with another person at a CSC.

How do I fill out the Virginia Driver’s License Replacement form?

Before attempting to fill out this form, ensure that you have everything needed to fill out this form, listed above. Make sure that all the information that you are about to place is completely accurate as this is a legal document and must be treated with utmost care.

Information for the Department of Elections
At the very top of the first page, you are asked to complete a small section and answer a few questions. This section is not required to be filled out to renew your driver’s license, but we will be guiding you through this portion in case you wish to fill this out.

On the top left of this section, you will be asked to indicate whether or not you are a U.S. citizen. Check the “Yes” or “No” box corresponding to your answer.

Right next to the previous question, you will be asked whether you wish to either vote or change your voter registration address. Once again, check the “Yes” or “No” box corresponding to your answer.

Information for the Virginia Transplant Council
Below the two previous questions is a question of whether or not you wish to become an organ, eye, and tissue donor. Simply check the box indicating “Yes” if you would like to become a donor, but if not, leave this question blank. This section, like the previous two questions, are not required to be answered if you wish to submit Form DL 1P.

Application Type
Here you will be asked to indicate your reason for filling out this form. Indicate whether you are applying for a driver’s license, learner’s permit and driver’s license, driver’s license with motorcycle, motorcycle license only, motorcycle learner’s permit, driver’s license with school bus endorsement, driver’s license testing for foreign diplomats, identification (ID) card, hearing impaired ID card, or emancipated minor’s ID card. Please check only one box.

Additionally, if you answered “driver’s license with motorcycle”, “motorcycle license only” or “motorcycle learner’s permit” in the previous question, you are asked to indicate your motorcycle classification. You will first be asked to indicate whether you are currently maintaining a current Virginia Motorcycle Classification.

You will then be asked if you want to add, upgrade, or transfer a motorcycle classification / obtain a Motorcycle Only License. Note that this may require additional testing.

You will then be asked to indicate whether you are applying for a license that applies to two-wheeled vehicles, three-wheeled vehicles, or both.

In the next segment, you will be asked to surrender your current license or identification card. If you are surrendering your license or ID card, check the corresponding box. In the event that you cannot surrender your current license or ID card, please indicate whether it was lost, stolen, or destroyed.

You will then be asked if you have a license, identification card, or learner’s permit from another state, U.S. territory, or foreign country. Check the“Yes or No” box that corresponds to your answer.

If you answered yes, you will be asked to provide your license/identification card number, issue date following the MM/DD/YYYY format, and state/country of acquisition.

Application Information
In this section, you will be asked to place the information required for the application of a license.

First, indicate your full legal name following the format of (last, first, middle, then suffix). In the box next to it, indicate your social security number (SSN). If you have not yet been issued an SSN, please indicate so by checking the box next to the space asking for it.

In the line below, place your date of birth following the MM/DD/YYYY format. You will then be asked if you wish to indicate your phone number, but take note that doing so is optional. You will then be asked to indicate whether you are male or female by checking the corresponding box. Following this, you will be asked to write down your weight in lbs., height in ft. and in., eye color, and hair color.

In the following line, place your current street address, followed by your apartment number, city, state, and zip code.

In the line below, if you changed your name previously, indicate your former legal name. Next to this, indicate your name of city or county of residence.

Next, if different from your residential address, indicate your current mailing address, apartment number, city, state, and zip code.

You will then be asked a series of “Yes or No” questions. Simply shade the box corresponding to your answer to the following questions:

  1. Do you wear glasses or contact lenses to operate a motor vehicle?
  2. Do you have a physical or mental condition that requires that you take medication?
  3. Have you ever had a seizure, blackout, or loss of consciousness?
  4. Do you have a physical condition that requires you to use special equipment while driving?
  5. Have you been convicted within the past ten years within the state or elsewhere of any offense resulting from your operation of, or involving, a motor vehicle? (Do not include parking tickets.
  6. Has your license or privilege to drive ever been suspended, revoked, or disqualified in this state or elsewhere, or is it currently suspended, revoked, or disqualified?

Note that if you answered “Yes” to any of the questions above, you will be asked to provide a brief explanation.

In the segment next to this, you will be asked to indicate whether any of the following applies to you:

  • Are currently an insulin-dependent diabetic
  • Has a speech impairment
  • Has a hearing impairment (license only)
  • Has an intellectual disability
  • Has autism spectrum disorder

Note that if you do have any of these, as previously mentioned, you will need to submit a physician’s statement.

Parent/Legal Guardian License Consent
If you are under the age of 18, you will need the permission of your parental figure or legal guardian. If they consent to your acquisition of a license, permit, or ID card, then they will need to check the boxes authorizing the issuance of either a learner’s permit/driver’s license or ID card.

They will also be asked to write their name in print, affix their signature, and indicate the date of signing in the MM/DD/YYYY format.

Additionally, you will be asked if you have ever been found to be not innocent of any offense in a Juvenile and Domestic Relations Court in this or any other state and to indicate so by checking the corresponding “Yes or No” box.

If you answered “Yes”, you will require court consent from a judge as they indicate whether in their opinion your request for a learner’s permit or license should be granted or not. They will be asked to indicate any remarks, their name, signature, and date of signing in the MM/DD/YY format.

Selective Service
If you are a male under the age of 26, you will be asked to check one of the following boxes. Failure to do so will result in a denial of your application.

  • I am already registered with Selective Service
  • I am a non-immigrant alien in the U.S. and not required to register
  • I authorize the DMV to forward to the Selective Service System personal information necessary to register me with Selective Service.

Note that by signing this, you are giving consent to being registered with Selective Service, if required by federal law. If you are under the age of 18, an appropriate adult must complete and sign below authorizing the DMV to send the information to Selective Service which will be used to register the applicant when you are 18 years old. The adult must check the corresponding box and affix their signature.

Veteran Indicator
You will be asked to indicate whether or not you want to add or keep the veteran indicator on your license / ID card. If you wish to have veteran indication, you will be required to complete Form DL 11, or the Virginia Veteran Military Service Certification form and provide proof that you are a military veteran.

Certification
At the very end of this form, you will be asked to place your name, signature, and date of signing in the MM/DD/YYYY format to verify all the information that you have placed within this form.

Once completed, congratulations, you have officially filled out Form DL 1P, or the Virginia Driver’s License Replacement Form.
