What is Form REG 227?

Form REG 227, Application for Duplicate or Transfer of Title, is a legal form in the state of California by the Department of Motor Vehicles (DMV). It serves two purposes; residents of California can use it to request a duplicate vehicle title if the original has been lost, stolen, or damaged beyond repair and to transfer vehicle ownership when the title is not available.

Take note that Form REG 227 cannot be used to release a lien on a vehicle with an Electronic Lien Title (ELT).

How to fill out Form REG 227?

Form REG 227 is a two-page form and contains eight sections. The sections to fill out will depend on the type of application.

Mark the box to determine the purpose of your application. You can select Duplicate Title or Transfer of Title with Duplicate.

For the application for the duplicate title, complete Sections 1 to 3. For the application for the transfer of title with duplicate, the seller needs to complete Sections 1 to 4 and the new owner needs to complete Sections 6 and 7, as needed.

Then, enter the following information, if applicable:

• Vehicle License Plate or Vehicle CF Number
• Vehicle/Hull Identification Number
• Year/Make of Vehicle or Vessel Builder

Section 1 — Registered Owner(s) of Record

True Full Name, Business Name, or Lessor

Enter the name of the registered owner as it appears on the title or registration. Use the following format: Last, First, Middle, Suffix.

Driver License/ID Card Number

Enter the driver’s license or identification card number of the registered owner.

State

Enter the state.

Co-Owner True Full Name

Enter the name of the registered co-owner as it appears on the title or registration., if applicable Use the following format: Last, First, Middle, Suffix.

Driver License/ID Card Number

Enter the driver’s license or identification card number of the registered co-owner, if applicable.

State

Enter the state.

Physical Residence of Business Address

Enter the full physical address or business address of the registered owner.

County of Residence or County Where Vehicle/Vessel is Principally Garaged

Enter the county where the vehicle or vessel is primarily garaged.

Mailing Address

Enter the full mailing address of the registered owner, if different from the physical residence or business address.

Section 2 — Legal Owner or Record (Lienholder/Title Holder)

Name of Bank, Finance Company, or Individual Having a Lien on This Vehicle

Enter the full legal name of the lienholder or titleholder.

Business or Residence Address

Enter the full business or residence address of the lienholder or titleholder.

Section 3 — Missing Title Statement

Take note that this section verifies that the title is missing. Issuance of a duplicate title cancels the original one.

Mark the box to determine the condition of the Certificate of Title of the vehicle or vessel. You can select:

• Lost
• Stolen
• Illegible/Mutilated — If you select this option, attach the old title.
• Not Received from Prior Owner
• Not Received from DMV — If you select this option, allow 30 days from the issue date.

Enter the full legal name of the lienholder or titleholder.

Signature of Owner

Provide the signature of the lienholder or titleholder.

Date

Enter the date when the owner signed the section.

Daytime Telephone Number

Enter a daytime telephone number.

By signing the missing title statement section, the signatory agrees to indemnify and save harmless the Director of Motor Vehicles for any loss suffered resulting from the issuance of the said duplicate Certificate of Title. He or she certifies or declares under the laws of the State of Califonia that the foregoing is true and correct.

Section 4 — Registered Owner(s) Release of Ownership and/or Interest

Provide the signature of the owner.

Enter the date the owner signed the section.

Enter a daytime telephone number.

If there are multiple owners, use the additional spaces provided.

By signing Section 4, the owner releases interest in the described vehicle or vessel. For a vehicle or vessel that has multiple owners, each owner is required to sign. For a vehicle or vessel that is owned by a company or business, the name of the company or business and an authorized representative’s countersignature are required.

Section 5 — Legal Owner of Record Release of Ownership and/or Interest

Purchase Price

Enter the amount the vehicle or vessel was purchased for.

Gift/Trade

If it was received as a gift or trade, mark the appropriate box.

Market Value

Enter the market value of the vehicle or vessel, if it was a gift or trade.

Enter the name of the new owner. Use the following format: Last, First, Middle, Suffix. If a business or lessor, enter the name.

Driver License/ID Card Number

Enter the driver’s license or identification card number of the registered co-owner, if applicable.

Enter the state.

If there are multiple new owners or lessees, use the additional spaces provided for the names, driver’s license or identification card numbers, and states.

Physical Residence of Business Address

Enter the full physical address or business address of the new owner.

County of Residence or County Where Vehicle/Vessel is Principally Garaged

Enter the county where the vehicle or vessel is primarily garaged.

Equipment Number

Enter the equipment number, if there is any.

Mailing Address

Enter the full mailing address of the new registered owner, if different from the physical residence or business address.

Lessee Address

Enter the full address of the lessee, if different from the given address above.

Vessel or Trailer Coach Principally Kept At

Enter the address or location of the vessel or trailer coach, if different from the physical address or business address above.

County

Enter the county.

Signatures of All New Owners

Provide the signature of the new owners.

Date

Enter the date the new owner or owners have signed.

Daytime Telephone Number

Enter the daytime telephone number of the new owner or owners.

Section 7 — New Legal Owner (Lienholder/Title Holder)

Only answer this section if applicable. If none, write “None.”

Provide the signature of the dealer agent of the vehicle.
